The Injury & Safety Report is a mandatory post trip legal document observers fill out to report any injuries they have incurred, illnesses they have had, or safety compromising situations such as no wheel watches they have encountered on an observed trip.
The purpose of the Injury @ Safety Report is to have a standardized legal format of reporting fishing vessel violations to partner agencies such as United States Coast Guard (USCG) and Occupational Safety and Health Administration (OSHA)
