Summary

Description

A Trip Ticket Summary Form must be filled out and sent to the department at least once a month.

Purpose

The trip ticket summary form documents the number of trip tickets filled out each time trip tickets are sent to the MDMR. If no trips are taken for a given month,

you are still required to send a monthly submission sheet indicating ?0? trips were obtained for that month.
