What is the Vessel and Gear Characterization Form?
The Gulf Shrimp Vessel and Gear Characterization Form is a required document to be completed and submitted with your application for a shrimp moratorium permit and with all annual permit renewals. This form is a standardized collection of information on shrimp vessel and gear data. The original copy of this form is to be mailed to the NOAA Fisheries Galveston Laboratory.
What is the Reporting (Annual Landings) Form?
The Gulf of Mexico Shrimp Federal Permit Reporting Form, also known as the Gulf Shrimp Landings Report Form, is a required document to be completed and submitted prior to your application for a shrimp moratorium permit renewal (annually). This form is a standardized collection of information on landings in the Gulf of Mexico, including bays, bayous, state inshore and offshore waters, and federal waters. The original copy of this form is to be mailed to the NOAA Fisheries Galveston Laboratory.
Where do I get my forms?
Two form packs are sent to the Gulf Shrimp permit holder. The two forms are the Gulf of Mexico Shrimp Federal Permit Reporting Form (Landings Report) and the Gulf Shrimp Vessel and Gear Characterization Form.

What if I have lost my annual landings receipts or trip tickets?
Please contact your dealer(s) directly or the State Trip Ticket Office for copies of your annual landings for the year being requested.
What if I am a new owner of a shrimp permit (SPGM) or transfer permit and do not have the previous owner's landings or gear information?
Please return both forms to the NOAA Galveston Lab with the new owner’s information:
